Kindergarten Registration

Documents Needed

• Proof of Age
  (Birth certificate, baptismal certificate, notarized statement from parent indicating date of birth, or duly attested transcript of the birth certificate.)
• Immunization Records
  (For more information about what immunizations are necessary, contact your doctor or the Montgomery County Health Department.)
• Proof of Residency
  (Dated and signed deeds, settlement papers or rental contracts, current tax statements or bills, current address on drivers license, current utility bill or other legal documents indicating permanent address. )
